PROCEDURES FOR GOVERNMENT PASSPORTS/DEPENDENTS

Guidelines For Official Passports
For an Official Passport you must be going to a country that requires you to have an Official Passport. If the command is unsure please go to www.fcg.pentagon.mil (this is a secure site)
NO FEE PASSPORT REQUIREMENTS:
DD1056 Filled out by the Command- Signed by Approving Official
2 Passport Photos (must be within 6 months)
MUST NOT BE WEARING UNIFORM
Size 2x2 with head being 1 to 1 3/8
MUST NOT BE WEARING GLASSES
MUST NOT HAVE Visible Tattoos
Proof of Citizenship- Original Birth Certificate (Must show the following to be acceptable)
Name of applicant or
Date and place of Birth
File Date should be within a year
Parents name
Signature of Register and Authorized Raised Seal
For information on how to obtain a certified copy of a US Birth certificate contact the Office of Vital Records in the State where the birth occurred or refer to: Cdc.gov/nchs/w2w.htm
Previous Passport or Most Current Blue Tourist Passport
Certificate of Naturalization
DS-11 Form Passport Application
Go to www.travel.state.gov Click on Passports and go to forms and fill out the DS-11 online. I need the form filled out online so a bar code is printed by your name. If you have a current passport you are submitting the form will change to a DS-82.
Copy of your ID Front and Back
Copy of your Orders or a memo request by the command
PREPAID FEDEX OR UPS ENVELOPE
If applying for dependents everyone must be present with all the forms including birth certificate, parents ID front and back
