July UTA Reporting Instructions

Reporting instructions for the July UTA will be dictated by your immediate leadership team. If you have questions or need further clarification, please contact your work center supervisor.

Additionally, Please review the questions below, and if you answer YES to any of them, please contact your supervisor immediately.

1.      Within the past 10 days, have you received a positive test result for an active COVID-19 infection? (This refers to a nasal swab. This does not include antibody/blood tests.)

2.      Within the last 14 days, have you been advised to stay home in quarantine due to an exposure to a positive case of COVID-19?

3.      Have you been tested for COVID-19, but results are still pending?

4.      Do you have any of the following symptoms that have not yet been evaluated? Fever, cough, shortness of breath, sore throat, muscle aches, new headache, or loss of taste/smell.

 

 

Also, if you were previously issued a cloth mask, bring it to the UTA, as the PHEO only has a limited amount left to issue to those who weren’t previously issued a mask.
